Position:

Social Care Assistant

Responsible to:

The Social Care Manager and to the Manager overall.

Responsibilities

HOURS:

20 hours per week over 4 days (10am to 330pm)

Alternate weekends


BASIC SKILLS REQUIREMENTS (including numeracy, literacy).


ABILITIES-

Experience with dementia care or care home environment an advantage

RESPONSIBILITIES-


GENERAL RESPONSIBILITIES OF THE POSITION

1. To undertake social activities and diversional activities within the home under the direction of the Head of Social Care – To promote occupation and well- being.

2. Supervise in lounges and corridors to help reduce falls and altercations

3. to record data from the activities provided.

4. Observance of health and safety requirements.

5. Complete all required mandatory training.

6. Promote best practice.


DAY TO DAY DUTIES-

1. Check diary for planned activities/entertainment on a daily basis.

2. Keep residents and staff up to date on planned activities and events.

3. Ensure birthdays are celebrated

4. Facilitate external entertainers and ensure all residents are invited

5. Keep updated in different aspects of activities

6. Facilitate and deliver group activities, individual activities and keep records of all activities provided.

7. Liaise with families to celebrate special occasions

8. Facilitate the spiritual needs of residents

9. Make photographic records of activities being enjoyed.

10. Help and assist residents with breakfast and lunches in the lounge and communal areas (not in rooms)

11. Help with afternoon tea in the lounge and communal areas (not in rooms) & help to assist residents in the lounge and communal areas at supper time (not in rooms).


TASKS FOR GROUP ACTIVITIES-

1. Be sure area to be used for activity is free

2. Set area up as needed for the type of activity planned.

3. Remind nursing staff of activities planned and residents who wish to attend.

4. Assist with transporting residents to the activity area

5. Follow the instructions for the activity as directed, checking the Risk Assessment, be aware of individuals cognitive, emotional, and sociable impairments, and adjust activity accordingly to promote inclusive and diverse practice.

6. Observe individual resident participation and responses during the activity.

7 Assist residents back to their living areas after the conclusion of the activity.

8. Notify nurses of resident’s behaviours which may indicate medical problems.


TASKS FOR INDIVIDUAL ACTIVITIES

1. Interact appropriately and effectively in a one to one activity according to the resident’s strengths and limitations.

2. Be aware of individuals cognitive, emotional, and sociable impairments, and adjust activity accordingly to promote inclusive and diverse practice.

3. Complete record sheets of activity and participation and responses of each resident and return them to the activity co-ordinator. Ensure all activity is documented in individual record folders and on CMS.

You may be asked to do additional jobs as needed to ensure the smooth running of the Home.
